ABSTRACT

Introducción:El síndrome de lisis tumoral (SLT) es una emergencia oncológica, que produce alteraciones en el metabolismo, causando manifestaciones clínicas y trastornos bioquímicos que ponen en peligro la vida del paciente.El objetivo del presente estudio fue identificar las características clínicas, de laboratorio y tratamiento del SLT, en pacientes pediátricos onco-lógicos, del Instituto del Cáncer SOLCA-Cuenca, en el periodo 2010 ­2020.Materiales y métodos:En este estudio se identificó las características del SLT, en pacientes pediátricos oncológicos, del Instituto del Cáncer SOLCA-Cuenca, en el periodo 2010 ­2020, a través de un estudio de tipo descriptivo-observacional.Resultados:Seincluyó 463 historias clínicas, en el cual se obtuvo que el SLT tuvo una frecuen-cia del 5.61 %, con predominio del sexo masculino (57.7%) y con una edad media de 7 ± 1.29 años. La presentación clínica más observada fue la deshidratación con náusea, vómito y dia-rrea (57.7%). Las alteraciones de laboratorio más frecuentes fueron la hiperuricemia y la hi-pocalcemia, con un 76.9 %y un 73.1 %respectivamente. La Leucemia linfoblástica aguda (LLA) fue el diagnósticooncológico con más casos (61.5 %). Los pilares del tratamiento fue-ron la hiperhidratación y el uso de alopurinol, utilizados en el 100% y un 80.8 %respectiva-mente.Conclusión:El SLT afectó más frecuentemente a varones, con diagnóstico de leucemia, ma-nifestaciones clínicas digestivas y alteraciones de laboratorio (hiperuricemia e hipocalcemia). El tratamiento empleado resultó eficaz y se basó en lo recomendado por la literatura médica


Introduction:Tumor lysis syndrome (TLS) is an oncological emergency that results in meta-bolic alterations, causing clinical manifestations and biochemical disorders that endanger pa-tients' lives. The objective of the present study was to identify the clinical, laboratory, and treat-ment characteristics of TLSsin pediatric oncology patients at the SOLCA-Cuenca Cancer Ins-titute from 2010­2020.Materials and methods: In this study, the characteristics of TLS were identified in pediatric oncology patients at the SOLCA-Cuenca Cancer Institute from 2010 to 2020 through a des-criptive observational study.Results: A total of463 medical records were included. TLSs were associated witha frequency of 5.61%, with a predominance of males(57.7%) and a mean age of 7 ± 1.29 years. The most commonclinical presentation was dehydration with nausea, vomiting, and diarrhea (57.7%). The most frequent laboratory alterations were hyperuricemia and hypocalcemia, with 76.9% and 73.1%,respectively. The oncological diagnosis was acutelymphoblastic leukemia (ALL) in most patients(61.5%). The pillars of treatment were hyperhydration and allopurinol, used in 100% and 80.8%, respectively.Conclusion: TLSsmore frequently affectmen with a diagnosis of leukemia, digestive clinical manifestations, orlaboratory alterations (hyperuricemia and hypocalcemia). The treatment used was effective and based on what the medical literature recommended

ABSTRACT

Objective:To determine whether the transepidermal water loss rate (TEWL) is correlated with the stratum corneum (SC) hydration level.Methods:Healthy children aged ≤ 17 years were enrolled from Medical Center for Public Health of Puning, 2 kindergartens and 2 primary schools, from October 2021 to June 2022. TEWL and SC hydration levels were measured on the left forearm and right anterior shank using a device for measuring skin physiological funcitons. Pearson correlation analysis was used to determine the correlations between TEWL and SC hydration levels in children of different ages and genders.Results:A total of 1 396 healthy children were enrolled, aged from 1 month to 17 years. Among them, 783 were male children and 613 were female children. In children aged 1 to < 12 months, no correlation was observed between TEWL and SC hydration levels on the forearms of male children, while TEWL was positively correlated with SC hydration levels on the anterior shanks of male children, as well as on the forearm and anterior shanks of female children ( r = 0.283, 0.404, 0.420, respectively, all P < 0.05) . In children aged 1 to 2 years, positive correlations were observed between the above two indicators on the anterior shanks of male children and forearms of female children ( r = 0.370, 0.419, respectively, both P < 0.01) , while there were no correlations between the two indicators on the anterior shanks of female children or forearms of male children. Positive correlations were observed between TEWL and SC hydration levels on both the forearms and anterior shanks of female children and the forearms of male children aged 3 to 5 years and 6 to 11 years ( r values ranging from 0.172 to 0.293, all P < 0.05) , but not on the anterior shanks of male children aged from 6 to 11 years. The group aged 12 to 17 years exhibited significantly positive correlations between TEWL and SC hydration levels on both the anterior shanks and forearms of male and female children ( r values ranging from 0.269 to 0.485, all P < 0.001) . Conclusion:SC hydration levels are positively correlated with TEWL on the anterior shanks and forearms of healthy children, and the degree of correlation tends to increase with age.

Hypertonic dehydration is prevalent in the elderly, increases the risk of adverse events, reduces quality of life, and increases hospitalization and mortality.Current methods of diagnosing dehydration, such as physical signs, urine tests, urea nitrogen/creatinine, bioelectrical impedance and saliva, are not suitable for the diagnosis of dehydration in the elderly.There is no gold standard for the diagnosis of hypertonic dehydration in the elderly.The European Society for Clinical Nutrition and Metabolism(ESPEN)recommends the direct measurement of plasma/serum osmotic pressure or using an osmotic pressure formula[osmolarity(mmol/L)=1.86 ×(Na + + K + )+ 1.15×glucose+ urea+ 14](all measured in mmol/L)for the diagnosis of hypertonic dehydration.

ABSTRACT

A dermatite atópica (DA) é uma doença cutânea inflamatória, crônica, comum, complexa e de etiologia multifatorial, que se manifesta clinicamente com prurido muitas vezes incapacitante, lesões recorrentes do tipo eczema, xerose e que pode evoluir para liquenificação. Embora o conhecimento sobre a sua fisiopatologia venham crescendo nos últimos anos, ainda as formas graves são frequentes e representam um desafio para o clínico. Para o presente guia realizou-se revisão não sistemática da literatura relacionada à DA grave refratária aos tratamentos habituais com o objetivo de elaborar um documento prático e que auxilie na compreensão dos mecanismos envolvidos na DA, assim como dos possíveis fatores de risco associados à sua apresentação. A integridade da barreira cutânea é um dos pontos fundamentais para a manutenção da homeostase da pele. Além dos cuidados gerais: evitação dos agentes desencadeantes e/ou irritantes, o uso de hidratantes, suporte emocional, entre outros, o uso de agentes anti-inflamatórios/imunossupressores de uso tópico e/ou sistêmico também foi revisado. A aquisição de novos agentes, os imunobiológicos e as pequenas moléculas, melhorou a terapêutica para os pacientes com formas graves de DA, sobretudo as refratárias aos tratamentos convencionais.


Atopic dermatitis is a chronic, common, and complex inflammatory skin disease with a multifactorial etiology. It manifests clinically with often disabling pruritus, recurrent eczema-like lesions, and xerosis, and can progress to lichenification. Although understanding of the disease's pathophysiology has been growing in recent years, severe forms are still frequent and represent a challenge for clinicians. A non-systematic review of the literature on severe atopic dermatitis refractory to conventional treatment was conducted to develop the present guide, whose purpose is to help clarify the mechanisms involved in the disease and possible risk factors. The integrity of the skin barrier is fundamental for maintaining skin homeostasis. In addition to general care, patients should avoid triggering and/or irritating agents and moisturizers and seek emotional support, etc.; the use of topical and/or systemic anti-inflammatory/immunosuppressive agents was also reviewed. New agents, immunobiologicals, and small molecules have led to a broader range of therapies for patients with severe forms of the disease, especially cases refractory to conventional treatment.

ABSTRACT

Introducción: Existen estrategias direccionadas a mejorar el rendimiento de los deportistas, algunas estableciendo recomendaciones respecto a la hidratación y reposición de líquidos, con el propósito de asegurar un balance hídrico adecuado en todas las etapas de la práctica deportiva. Sin embargo, existe una carencia de instrumentos adecuados y confiables que permitan explorar esta temática. Objetivo: La presente investigación tiene como objetivo validar por contenido y evaluar la fiabilidad del instrumento "Conocimiento, actitud y comportamiento respecto a la hidratación y reposición de líquidos" en deportistas adolescentes y adultos jóvenes en Chile. Métodos: La adaptación cultural se realizó a través del método Delphi con un panel de expertos en el área, para la evaluación de la fiabilidad se aplicó el cuestionario de manera online a una muestra de 384 deportistas de diferentes disciplinas en dos oportunidades para determinar la estabilidad temporal. Resultados: Todos los ítems lograron porcentajes de acuerdo superior al 80% según el estadístico V de Aiken, en relación con la fiabilidad se obtuvo un Alfa de Cronbach de 0,78 lo que evidencia una consistencia interna aceptable, adicionalmente se obtuvo un Coeficiente de Correlación intraclase de 0,64 entre las dos aplicaciones del cuestionario presentando una fiabilidad regular a buena. Conclusión: Este cuestionario puede ser empleado como una herramienta confiable para ser utilizada en futuras investigaciones en el área de la nutrición deportiva, además como un instrumento que aporta al quehacer de distintos profesionales para apoyar a planificar y evaluar estrategias que propendan al mejoramiento del rendimiento deportivo.


Introduction: There are strategies aimed at improving the performance of athletes, some of which establish recommendations regarding hydration and fluid replacement, with the purpose of ensuring an adequate water balance at all stages of sports practice. However, there is a lack of adequate and reliable instruments to explore this issue. Objective: The present research aims to validate the content and evaluate the reliability of the instrument "Knowledge, attitude and behavior regarding hydration and fluid replacement" among adolescent and young adult athletes in Chile. Methods: The cultural adaptation was carried out through the Delphi method with a panel of experts in the area. For the evaluation of reliability, the questionnaire was applied online to a sample of 384 athletes from different disciplines on two occasions to determine temporal stability. Results: All items achieved percentages of agreement higher than 80% according to Aiken's V statistic, in relation to reliability a Cronbach's Alpha of 0.78 was obtained, which shows an acceptable internal consistency, additionally an intraclass Correlation Coefficient of 0.64 was obtained between the two applications of the questionnaire presenting a regular to good reliability. Conclusion: This questionnaire can be used as a reliable tool for future research around sports nutrition, as well as an instrument that contributes to the work of different professionals to support the planning and evaluation of strategies aimed at improving sports performance.

Purpose: We aimed to assess the impact of drinking water (500 and 1000 mL) on corneal biomechanics and determine the level of association between changes in intraocular pressure and variations in the different biomechanical properties of the cornea. Methods: A total of 39 healthy young adults ingested either 1000 mL (n = 21) or 500 mL (n = 18) of tap water in 5 min. The CorVis ST system was used to assess corneal biomechanics at baseline and at 15, 30, and 45 min after water ingestion. Results: Water drinking induced statistically significant changes in the deformation amplitude (P < 0.001, ?� = 0.166), highest concavity time (P = 0.012, ?� = 0.093), peak distance (P < 0.001, ?� = 0.171), time and velocity of the first applanation (P < 0.001, ?� = 0.288 and P = 0.016, ?� = 0.087, respectively), and time and velocity of the second applanation (P = 0.030, ?� = 0.074 and P = 0.001, ?� = 0.132, respectively), being independent of the amount of water ingested (P > 0.05 in all cases). There were significant associations between changes in intraocular pressure and some parameters of corneal biomechanics. Conclusion: Small variations in whole?body hydration status alter different biomechanical properties of the cornea, with these changes being associated with intraocular pressure levels. These findings indicate that whole?body hydration status can be considered for the diagnosis and management of different ocular conditions.

Despite numerous studies related to dehydration there is still a lack of scientific literature presenting hydration status and fluid intake of judo athletes during different periods. Therefore, the aim of this study was to investigate, fluid intake, hydration status and body weight changes of young judo athletes during a typical day of training in preparation period. Twenty-two young judo athletes (age: 12 ± 0.7 y, experience: 3.5 ± 1.1) voluntarily participated in this study. Hydration status and weight were examined in the morning, before and immediately after the training. All athletes trained 90 min and they consumed fluids ad libitum during the exercise. According to morning urine specific gravity (USG) values, 81.2% of the athletes were dehydrated while only 18.8% of the athletes were euhydrated. Pre-training urine measurements showed that 63.64% of the athletes presented dehydration and 77.27% of the athletes completed the training in dehydrated condition despite fluid availability during the training. Mean body weight loss during training was -0.64 ± 0.66%. It can be concluded that young judo athletes presented high prevalence of dehydration as indicated by USG values. Most of the athletes were dehydrated during a typical training day and completed the training in more dehydrated conditions compared to pre training values despite ad libitum fluid intake. It is of great importance to evaluate hydration status of the athletes before training to refrain from common practice of fluid restriction for weight loss and adverse effects of a persistent state of fluid deficit on physical and health related state.

ABSTRACT

Malnutrition and dehydration are prevalent in the elderly poplulation, and obesity is also a growing problem, which pose a serious challenge to the nutritional management in geriatrics. In order to better guide clinical practice, the European Society for Clinical Nutrition and Metabolism (ESPEN) published the practical guideline on clinical nutrition and hydration in geria-trics on March 5, 2022. This guideline provides 82 recommendations on clinical nutrition and hydration in geriatrics based on clinical practicability, covering basic problems and general prin-ciples, prevention and treatment of malnutrition/nutri-tional risk, prevention and treatment of specific diseases, as well as prevention and treatment of obesity, along with flow-charts, hoping to be convenient for doctors, nutritionists and nurses to use in clinical practice.

para llevar a cabo esta investigación, se revisó la literatura sobre el caso de Terri Schiavo, paciente que se encontraba en estado vegetativo persistente y quien falleció luego de dos semanas, después de que se le suspendiera su alimentación e hidratación; se validó su diagnóstico y se indagó si fue una paciente terminal, así como también se verificó la evidencia disponible, en relación con la hidratación y nutrición artificial en este tipo de pacientes, para determinar la concordancia de estas medidas. Esta información fue analizada desde la perspectiva nutricional y bioética; en la búsqueda bibliográfica se consultaron las bases de datos Scopus, Scielo y PubMed, con los criterios de búsqueda nutrición e hidratación artificial en pacientes terminales y de pronóstico incierto. Estos hallazgos fueron analizados con el modelo de proporcionalidad terapéutica de Calipari. Por lo anterior, se determinó que la nutrición e hidratación artificial configuraban tratamientos de carácter obligatorio u optativo para Terri. Sin embargo, pese a que no existe información concluyente sobre la nutrición e hidratación artificial en pacientes terminales, ni de pronóstico incierto, se recomienda la evaluación caso a caso de parte del equipo médico, para determinar la proporcionalidad de estos procedimientos en conjunto con el paciente y su familia. Cabe resaltar que son necesarios más estudios para proporcionar mejor evidencia que permita contar con elementos objetivos para una mejor toma de decisiones.


to carry out this research, the literature was reviewed on the case of Terri Schiavo, a patient who was in a persistent vegetative state and who died after two weeks, after her nutrition and hydration were suspended; her diagnosis was validated, and it was investigated whether she was a terminal patient, as well as the available evidence, was verified, in relation to artificial hydration and nutrition in this type of patients to determine the concordance of these measures. This information was analyzed from the nutritional and bioethical perspective; in the bibliographic search, the Scopus, Scielo, and PubMed databases were consulted with the search criteria nutrition and artificial hydration in terminal patients and patients with uncertain prognosis; these findings were analyzed with Calipari's therapeutic proportionality model. Therefore, it was determined that artificial nutrition and hydration are mandatory or optional treatments for Terri. However, there is no conclusive information on artificial nutrition and hydration in terminally ill patients, nor is the prognosis uncertain, and a case-by-case evaluation by the medical team is recommended to determine the proportionality of artificial nutrition and hydration, together with the patient and family. It should be emphasized that more studies are needed to provide better evidence to provide objective elements for better decision-making.


Para realizar esta pesquisa, foi revisada a literatura sobre o caso de Terri Schiavo, paciente que se encontrava em estado vegetativo persistente e que faleceu após duas semanas depois de sua alimentação e hidratação terem sido suspensas. Foi avaliado seu diagnóstico e questionado se foi uma paciente terminal, bem como verificada a evidência disponível quanto à hidratação e à nutrição artificiais nesse tipo de pacientes para determinar a concordância dessas medidas. Essa informação foi analisada sob a perspectiva nutricional e bioética; na busca bibliográfica, foram consultadas as bases de dados Scopus, SciELO e PubMed, com os critérios de busca "nutrição e hidratação artificiais em pacientes terminais e de prognóstico incerto". Esses achados foram analisados com o modelo de proporcionalidade terapêutica de Calipari. Assim, foi determinado que a nutrição e a hidratação artificiais configuram tratamentos de caráter obrigatório ou opcional para Terri. Contudo, não existe informação conclusiva sobre a nutrição e a hidratação artificiais em pacientes terminais, nem em pacientes com prognóstico incerto. É recomendada a avaliação por parte da equipe médica caso a caso para determinar a adequação da nutrição e da hidratação artificiais em conjunto com o paciente e sua família. Cabe ressaltar que mais estudos são necessários para proporcionar melhor evidência que permita contar com elementos objetivos para uma melhor tomada de decisões.

RESUMEN: La Bioimpedancia Eléctrica (BIA), al ser una técnica no invasiva pero de elevada precisión, se ha convertido en la actualidad en una herramienta valiosa para determinar la composición corporal en militares, facilitado el control de las distintas variables que se asocian a cada especialidad. El objetivo del presente estudio fue describir el perfil de composición corporal en militares de elite al momento de finalizar un curso de especialización. Participaron 11 militares con un rango de edad entre 22 y 29 años. Se evalúo la composición corporal a través de BIA, inmediatamente después de finalizado un curso de especialización para militares de élite. Las evaluaciones en los militares sobre las variables de la composición corporal a través de BIA mostraron: peso corporal de 84,3 ± 4,52 kg, talla 1,78 ± 0,06 m, índice de masa corporal (IMC) 26,5 ± 1,09, tejido adiposo de 13,7 ± 3,65 %, tejido muscular 49,5 ± 2,34 %, masa libre de grasa 72,7 ± 5,23 kg y 53,2 ± 3,78 l de agua corporal total. Conclusiones: Los militares de elite presentaron elevados niveles de masa libre de grasa, tejido muscular y bajos niveles de tejido adiposo lo que favorece el desarrollo de las actividades militares especializadas y disminuye el riesgo de lesiones. Los datos aquí recogidos sirven como marco de referencia para futuros estudios.


SUMMARY: Electrical bioimpedance (BIA), being a non- invasive technique but with high precision, has become a valuable tool for determining body composition in the military, facilitating the control of the different variables associated with each specialty. The aim of the present study was to describe the body composition profile of elite military personnel at the end of a specialization course. Eleven military personnel between 22 and 29 years of age participated in the study. Body composition was assessed by BIA immediately after completion of a specialization course for elite military personnel. Assessments in the military on body composition variables through BIA showed: body weight of 84.3 ± 4.52 kg, height 1.78 ± 0.06 m, body mass index (BMI) 26.5 ± 1.09, adipose tissue of 13.7 ± 3.65 %, muscle tissue 49.5 ± 2.34 %, fat free mass 72.7 ± 5.23 kg and 53.2 ± 3.78 l of total body water. Conclusions: Elite military personnel presented high levels of fat free mass, muscle tissue and low levels of adipose tissue which favors the development of specialized military activities and decreases the risk of injury. The data collected here serve as a frame of reference for future studies.

A hidratação cutânea ocorre, em parte, pelos componentes do Fator de Hidratação Natural (NMF), originados da degradação da filagrina, sendo alguns exemplos o ácido pirrolidona-carboxílico (PCA), o ácido urocânico (UCA) e a histidina (His). Estes estão presentes no estrato córneo (EC). O objetivo deste projeto de pesquisa foi determinar, por cromatografia líquida de alta eficiência (CLAE), o PCA, o UCA e a His no estrato córneo de participantes obtido por tape stripping em função do fototipo e idade do participante da pesquisa. Participantes foram selecionados em função da idade acima de 18 anos, ambos os gêneros e fototipo da pele entre I a VI, de acordo com a classificação de Fitzpatrick. As amostras do EC foram obtidas do antebraço volar por tape stripping e irradiadas artificialmente. A cromatografia líquida de alta eficiência (CLAE) foi eficaz para separação e quantificação adequada das substâncias químicas His, PCA e os isômeros de UCA (trans-UCA e cis-UCA) no estrato córneo dos participantes. O método apresentou-se seletivo e ausente de interferentes, ademais, possuiu linearidade e limites de detecção e quantificação compatíveis com os objetivos dessa investigação. No fototipo I, os níveis de His foram menores em comparação aos demais grupos. Ademais, os níveis dessa mesma substância não apresentaram diferença entre as faixas etárias. Em função da irradiação das amostras, o montante de His aumentou em todos os fototipos. Os níveis de PCA apresentaram-se menores após a irradiação em todos os fototipos de pele. Ainda, as concentrações do PCA foram mais elevadas na faixa etária de 46 a 55 anos de idade. Os níveis de concentração do isômero cis-UCA foram maiores nos participantes com fototipo III, após a irradiação UV. Os níveis de concentração do isômero trans-UCA diminuíram após a irradiação, de forma proporcional à formação de cis-UCA em todos os fototipos. A faixa etária de 46-55 anos de idade obteve níveis significativamente menores de trans-UCA e cis-UCA


Cutaneous hydration occurs, in part, by the components of the Natural Hydration Factor (MFN), originating from the degradation of filagrina, some examples being pyrrolidone-carboxylic acid (PCA), urocanic acid (UCA) and histidine (His). These are present in the stratum corneum (SC). The objective of this research project was to determine, by high efficiency liquid chromatography (HPLC), the PCA, UCA and His in the stratum corneum of participants obtained by tape stripping due to the phototype and age of the research participant. Participants were selected according to age above 18 years, both genders and skin phototype between I and VI, according to Fitzpatrick's classification. The SC samples were obtained from the volar forearm by tape stripping and artificially irradiated. High efficiency liquid chromatography (HPLC) was effective for the separation and proper quantification of the chemicals His, PCA and UCA isomers (trans-UCA and cis-UCA) in the stratum corneum of the participants. The method was selective and absent from interfering, in addition, it had linearity and limits of detection and quantification compatible with the objectives of this investigation. In phototype I, His levels were lower compared to the other groups. Moreover, the levels of this same substance showed no difference between age groups. Due to the irradiation of the samples, the amount of His increased in all phototypes. PCA levels were lower after irradiation in all skin phototypes. Furthermore, PCA concentrations were higher in the age group from 46 to 55 years of age. The concentration levels of the cis-UCA isomer were higher in participants with phototype III after UV irradiation. The concentration levels of the trans-UCA isomer decreased after irradiation, proportionally to the formation of cis-UCA in all phototypes. The age group 46-55 years of age obtained significantly lower levels of trans-UCA and cis-UCA

ABSTRACT

Liposomes with precisely controlled composition are usually used as membrane model systems to investigate the fundamental interactions of membrane components under well-defined conditions. Hydration method is the most common method for liposome formation which is found to be influenced by composition of the medium. In this paper, the effects of small alcohol (ethanol) on the hydration of lipid molecules and the formation of liposomes were investigated, as well as its coexistence with sodium chloride. It was found that ethanol showed the opposite effect to that of sodium chloride on the hydration of lipid molecules and the formation of liposomes. The presence of ethanol promoted the formation of liposomes within a certain range of ethanol content, but that of sodium chloride suppressed the liposome formation. By investigating the fluorescence intensity and continuity of the swelled membranes as a function of contents of ethanol and sodium chloride, it was found that sodium chloride and ethanol showed the additive effect on the hydration of lipid molecules when they coexisted in the medium. The results may provide some reference for the efficient preparation of liposomes.

ABSTRACT

It has been speculated that dry skin results from dysfunction of epidermal permeability barrier. But so far, there is no sufficient evidence to support this speculation. In fact, dry skin indicates low levels of stratum corneum hydration. Stratum corneum hydration levels are primarily determined by the content of natural moisturizers in the skin, while epidermal permeability barrier is mainly regulated by intercellular lipids and structural proteins in the stratum corneum. If dry skin is due to a defective permeability barrier, stratum corneum hydration levels should be inversely correlated with transepidermal water loss (TEWL) , an indicator of epidermal permeability barrier function. But no negative correlation has been demonstrated between stratum corneum hydration levels and TEWL in either normal human skin, ichthyoses lesions of patients, or sebaceous gland-deficient mice in a number of studies. In contrast, a positive correlation between stratum corneum hydration levels and TEWL was observed in normal human skin. Taken together, a line of evidence suggests that dry skin unlikely indicates epidermal permeability barrier dysfunction.

Abstract Introduction: Urinary tract symptoms and infection have been associated with occupational factors that impact hydration habits particularly in women. We compared self-reported urinary symptoms and infection and hydration habits between nurses and other occupations in dialysis units. Methods: Cross-sectional study. Participants worked in five nephrology centers in Brazil and answered an online questionnaire comprising questions regarding urinary tract symptoms and infection episodes in the preceding year; data on usual daily beverage intake, urine frequency, and urine color according to a urine color chart were also collected, as well as perceptions of water access and toilet adequacy at work. Results: We included 133 women (age=36.9±9.5 years). The self-reported usual daily beverage intake was 6.6±2.9 cups/day (~1320 mL), daily urine frequency was 5.4±2.1, and urine color chart score: 3.0±1.2. Nurses (N=66/49.6%) reported higher prevalence of burning sensation (50 versus 27%; P<0.001), urinary urgency (42 versus 21%; P<0.001), and infection (42% versus 25%; P=0.04) as well as lower liquid intake (6.0±2.6 versus 7.3±3.0 cups/day; P=0.01) than controls. Forty four percent of nurses reported being able to drink when thirsty "always" and "most of the time" versus 93% of the control group. Conclusion: Dialysis female nurses reported lower beverage intake and higher prevalence of symptoms and infection than other occupations in the same environment. Interventions to improve hydration can potentially decrease urinary problems in this population.


Resumo Introdução: Sintomas e infecções do trato urinário têm sido associados a fatores ocupacionais que afetam hábitos de hidratação, particularmente em mulheres. Comparamos sintomas e infecções urinárias autorrelatados e hábitos de hidratação entre técnicas de enfermagem e outras ocupações em unidades de diálise. Métodos: Estudo transversal. As participantes trabalhavam em cinco centros de nefrologia no Brasil e responderam um questionário online contendo perguntas sobre sintomas do trato urinário e episódios de infecção no ano anterior; também foram coletados dados sobre a ingestão diária habitual de bebidas, frequência e cor da urina de acordo com uma tabela de cores da urina, assim como percepções sobre o acesso à água e adequação das instalações sanitárias no trabalho. Resultados: Incluímos 133 mulheres (idade=36,9±9,5 anos). Em média, as participantes relataram ingestão de bebidas de 6,6±2,9 copos/dia (~1320 mL), frequência urinária de 5,4±2,1 vezes/dia e escore da tabela de cores da urina de 3,0±1,2. Técnicas de enfremagem (N=66/49,6%) relataram maior prevalência de sensação de ardor (50 versus 27%; P<0,001), urgência urinária (42 versus 21%; P<0,001), e infecção (42% versus 25%; P=0,04), bem como menor ingestão de líquidos (6,0±2,6 versus 7,3±3,0 copos/dia; P=0,01) do que controles. Quarenta e quatro por cento das técnicas de enfermagem relataram poder beber quando têm sede "sempre" e "na maioria das vezes" contra 93% do grupo controle. Conclusão: Técnicas de enfermagem de unidades de diálise relataram menor ingestão de bebidas e maior prevalência de sintomas e infecção do que outras ocupações no mesmo ambiente de trabalho. Intervenções para melhorar a hidratação podem diminuir problemas urinários nesta população.

Abstract Purpose: Hydration has been shown to play a pivotal role in sport. Soccer is a team sport in which the integrity of all players is vital for team performance; thus, individual player hydration status is important. The primary aim of this study was to investigate the hydration status of female collegiate soccer players during regular season. Methods: Sixteen collegiate female soccer players (age: 20.4 ± 0.8 years; height: 163.6 ± 6.9 cm; weight: 65.3 ± 12 kg) provided urine over 9 days to monitor their hydration status. Hydration was determined by urine specific gravity (Usg). Usg was analyzed in the morning (AM) and in the afternoon before practice/game (PM). Results: All 16 players were at least minimally hypohydrated (Usg > 1.010) in the AM on 5 of the 9 days tested. Players had significantly higher Usg values in the AM as compared to PM (F(1,250) = 23.09; p < 0.0001); however, there was no significant time*day interaction (F(1,250) = 1.98; p = 0.16). Conclusion: Data show a high prevalence of hypohydration occurring in this specific population. This sub-optimal hydration status could be a cause for concern in terms of overall performance. Efforts should be made to integrate hydration interventions and daily monitoring to minimize hypohydration in players.


Resumen Propósito: Se ha demostrado que la hidratación juega un papel fundamental en el deporte. El fútbol es un deporte de equipo en el que la integridad de todos los jugadores es vital para su rendimiento; por lo tanto, el estado de hidratación de cada jugador es importante. El objetivo principal de este estudio fue investigar el estado de hidratación de las jugadoras universitarias de fútbol durante la temporada regular. Métodos: Dieciséis jugadoras universitarias de fútbol (edad: 20.4 ± 0.8 años; estatura: 163.6 ± 6.9 cm; peso: 65.3 ± 12 kg) proporcionaron muestras de orina durante 9 días para controlar su estado de hidratación, la cual se determinó por la gravedad específica de la orina (Usg). La Usg se analizó en la mañana (AM) y en la tarde antes del entrenamiento o el juego (PM). Resultados: Las 16 jugadoras estaban al menos mínimamente hipohidratadas (Usg > 1.010) en la AM en 5 de los 9 días medidos. Las jugadoras tuvieron valores Usg significativamente más altos en AM en comparación con PM (F (1,250) = 23.09; p < 0.0001); sin embargo, no hubo interacción significativa de tiempo * día (F(1,250) = 1.98; p = 0.16). Conclusión: los datos muestran una alta prevalencia de hipohidratación en esta población específica. Este estado de hidratación subóptimo podría ser motivo de preocupación en términos de rendimiento general. Se deberían hacer esfuerzos para integrar las intervenciones de hidratación y el monitoreo diario para reducir al mínimo la hipohidratación en las jugadoras.

O futebol é um esporte coletivo no qual a integridade de todos os jogadores é vital para o desempenho da equipe; portanto, o estado de hidratação de cada jogador é importante. O objetivo principal deste estudo foi investigar o estado de hidratação de universitárias jogadoras de futebol do sexo feminino durante a temporada regular. Métodos: Dezesseis universitárias jogadoras de futebol (idade: 20,4 ± 0,8 anos; altura: 163,6 ± 6,9 cm; peso: 65,3 ± 12 kg) forneceram amostras de urina por 9 dias para monitorar seu estado de hidratação. A hidratação foi determinada pela gravidade específica da urina Usg. Sua Usg foi analisado pela manhã (matutino) e à tarde (vespertino) antes do treino ou jogo. Resultados: As 16 jogadoras estavam minimamente hipohidratadas (Usg > 1.010) no período matutino em 5 dos 9 dias medidos. As jogadoras apresentaram valores de Usg significativamente maiores no período matutino em comparação ao vespertino (F (1.250) = 23,09; p <0,0001); no entanto, não houve interação tempo * dia significativa (F (1.250) = 1,98; p = 0,16). Conclusão: os dados mostram uma alta prevalência de hipohidratação nesta população específica. Este estado de hidratação abaixo do ideal pode ser uma preocupação em termos de desempenho geral. Devem ser feitos esforços para integrar intervenções de hidratação e monitoramento diário para minimizar a hipohidratação em jogadoras.

SUMMARY: The main purpose of this study was to determine the body composition and anthropometric reference values of male and female junior Croatian taekwondo competitors according to official weight category. Additional aim of the study was to identify possible differences in variables between the weight categories. The sample comprised 247 junior participants of the national taekwondo championships divided into male (N=121) and female (N=126). The variable sample was defined by a set of 8 measurements: Body height (cm), Body mass (kg), Body mass index, Body fat (%), Body fat (kg), Muscle mass (kg), Lean body mass (kg) and Total body water (%). Results confirm previous studies proving that younger taekwondo practitioners have higher levels of body fat than their more experienced peers (seniors). Significant differences were only found between lean and heavy categories. Male junior taekwondo athletes show total body water percentages from 57.4 to 74.8 %, with significantly lower percentages in higher weight categories, while female athletes in heigher categories demonstrate a worrying level of hydration status, therefore endangering their performance and overall health. As the first paper that investigates the reference values of junior taekwondo athletes but based on official weight categories it is especially important as each weight category warrant different body composition requirements, as seen from significant differences across different variables. Future studies should include motor abilities inspection throughout the weight categories and possible use of calculation of metabolic age.


RESUMEN: El objetivo principal de este estudio fue determinar la composición corporal y los valores de referencia antropométricos de los competidores croatas masculinos y femeninos de taekwondo junior según la categoría oficial de peso. Además, se analizó identificar las posibles diferencias en las variables entre las categorías de peso. La muestra estuvo compuesta por 247 participantes junior de los campeonatos nacionales de taekwondo divididos en hombres (N = 121) y mujeres (N = 126). La muestra variable se definió mediante un conjunto de 8 medidas: altura corporal (cm), masa corporal (kg), índice de masa corporal, grasa corporal (%), grasa corporal (kg), masa muscular (kg), masa corporal magra (kg) y agua corporal total (%). Los resultados confirmaron estudios previos que demuestran que los practicantes de taekwondo más jóvenes tienen niveles más altos de grasa corporal que sus pares más experimentados (personas mayores). Solo se encontraron diferencias significativas entre las categorías magra y pesada. Los atletas masculinos de taekwondo junior muestran porcentajes de agua corporal total del 57,4 al 74,8 %, con porcentajes significativamente más bajos en las categorías de peso más alto, mientras que las atletas femeninas en las categorías más altas, demuestran un nivel preocupante de estado de hidratación, poniendo en peligro su rendimiento y salud en general. Como el primer artículo que investiga los valores de referencia de los atletas de taekwondo junior, pero basado en categorías de peso oficiales, es especialmente importante ya que cada categoría de peso garantiza diferentes requisitos de composición corporal, como se puede observar en las diferencias significativas entre las diversas variables. Los estudios futuros deberían incluir un análisis de las habilidades motoras en todas las categorías de peso y el posible uso del cálculo de la edad metabólica.

ABSTRACT

Background:Oligohydramniosmeans a low level of amniotic fluid during pregnancy. It is defined by an amniotic fluid index that is below 5th percentile for the gestational age. It may cause IUGR, fetal anomalies, malpresentations and fetal distress in labour. Oral or intravenous fluid therapy may increase the amount of amniotic fluid and may improve perinatal outcome. Aims And Objectives:To study efficacy and perinatal outcome of hydrotherapy in patients with oligohydamnios. Material & Methods:A prospective observational study wascarried out at tertiary care centre for 6 months. 30 cases of oligohydramnios were enrolled in this study. Results:In present study, maximum (50%) patients admitted for oligohydramnios were between 28 –32 weeks gestational age and 66% were having severeoligohydramnios. After giving hydration therapy, difference in AFI, was 2.57±0.13, which was statistically significant. 83.3% patients were delivered after 36 weeks and 80% were delivered by normal vaginal delivery. 53.3% babies were live healthy, whereasothers needed resuscitation and admitted to NICU. Conclusion:Oligohydramnios has higher impact on perinalal outcome due to antenatal and intrapartum complication. Because of early diagnosis by ultrasonography or clinical analysis, fluid therapy is an option to increase amniotic fluid volume and so perinatal outcome may be improved.

RESUMO Objetivo Analisar o efeito imediato da hidratação de superfície laríngea associado à técnica de vibração sonorizada de língua (TVSL) em cantores. Método Participaram 30 cantores, sem queixas vocais ou alterações laríngeas, divididos em grupo controle (GC) e experimental (GE). O GC realizou a TVSL por cinco minutos. O GE foi submetido à nebulização de 3 ml de solução salina seguido da TVSL por cinco minutos. Foram realizadas autoavaliação vocal, análise acústica e avaliação perceptivoauditiva nos momentos pré (PréTVSL) e pós (PTVSL) no GC e no momento pré (PréHTVSL), pós hidratação (PH) e pós hidratação+TVSL (PHTVSL) no GE. Na autoavaliação foram avaliados: qualidade, estabilidade, rouquidão e intensidade vocal. Os parâmetros acústicos analisados foram Frequência Fundamental; Jitter%; Shimmer%, Noise-to-harmonic Ratio e Cepstral Peak Prominence-Smoothed (CPPs). A avaliação perceptivoauditiva foi realizada por uma fonoaudióloga experiente. Resultados Na comparação dos resultados da autoavaliação, entre os grupos, observou-se melhora da percepção de estabilidade e intensidade vocal no PTVSL (GC) em relação ao PH (GE). Na comparação entre os momentos do GE houve diferença estatística na sensação de intensidade vocal, apontando melhor resultado para PHTVSL. Não houve diferença estatística entre os grupos investigados na avaliação perceptivoauditiva e na análise acústica. Conclusão A hidratação laríngea de superfície não potencializa o efeito da TVSL em cantores em condição natural de hidratação com uso de 3ml de nebulização. Para os profissionais da voz com grande demanda vocal, a hidratação de superfície pode ser introduzida durante a utilização da voz, para manutenção da qualidade vocal, sem perda de sua qualidade.


ABSTRACT Purpose To analyze the immediate effect of laryngeal surface hydration associated with the performance of Tongue Trills (TT) on singers. Methods Thirty singers without vocal complaints or laryngeal alterations divided into control (CG) and experimental (EG) groups. The CG performed the TT for five minutes. The EG was submitted a nebulization with 3 ml of saline solution followed by TT for five minutes. Voice self-assessment, acoustic analysis and perceptual assessment were performed at Pre (Pre TT) and post (PTT) moments in CG and pre (Pre TT), post hydration (PH) and post hydration + TT (PHTT) in GE. In the self-assessment were evaluated quality, stability, vocal intensity and hoarseness. There were extract the values of the Fundamental frequency; Jitter%; Shimmer%, Noise-to-harmonic Ratio e Cepstral Peak Prominence-Smoothed (CPPs) in the acoustic analyze. The perceptual evaluation was performed by an experienced speech therapist. Results Comparing the results of self-assessment between groups showed improvement in the perception of stability and vocal intensity in the PTT (CG) in relation to PH (EG). Comparison between the EG moments showed a statistical difference in the vocal intensity perception, indicating a better results for PHTT. There was no statistical difference between the groups investigated in the perceptual assessments and acoustic analysis. Conclusion Surface laryngeal hydration does not potentiate the effect of TT on naturally hydrated singers with 3ml nebulization. For voice professionals with high vocal demand, surface hydration can be introduced during voice use to maintain vocal quality.

@#Introduction: This study aimed to evaluate the short-term efficacy of two comparable formulation of dual-polymer artificial tears: Systane Hydration preservative (SH) and non-preservative (SHUD) in 60 minutes observation period compared to normal saline. Methods: Fifty participants involved in this prospective, double-masked randomised study. Viscosity and pH of artificial tears were evaluated using rheometer and digital pH-meter prior to tear film assessment. Tear break-up time (TBUT) and tear meniscus height (TMH) were measured at baseline, 5, 15 and 60 minutes after instillation. Tear ferning pattern (TFP) were compared between baseline and 60 minutes after instillation. One-way analysis of variance (ANOVA) and Independent T-test were used to evaluate the effects of SH and SHUD after instillation and comparison between each specific time-interval respectively. P-value of 0.05 was set as the level of significance. Results: The viscosity of SH and SHUD was 26.7cP and 32.73cP re-spectively with pH of 7.85 (SH) and 7.74 (SHUD). Both artificial tears showed significant increment in TBUT between baseline and 15 minutes (SH:5.82±1.063, p=0.01; SHUD:6.02±0.979, p<0.001), and 60 minutes (SH:6.22±0.616, p<0.001; SHUD:6.34±0.658, p<0.001). SHUD demonstrated significant increment in TMH at every measurement taken (0.1996±0.02449, p<0.001 at 5min, 0.2038±0.02276, p<0.001 at 15min and 0.2068±0.02094, p<0.001 at 60min). Likewise, in SH group, significant increment in TMH at 15 minutes (0.1994±0.02325, p<0.001) and 60 minutes (0.2012±0.02379, p<0.001) were noted. Both groups revealed improvement in TFP (both, p<0.001) at 60 minutes. No significant im-provement was noted in control group. Conclusion: Improvement in TMH was prominently faster in SHUD than SH, although both TBUT and TFP revealed comparable tears quality between both artificial tears.

INTRODUCTION@#Patients receiving intravenous iodinated contrast media for computed tomography (CT) are predisposed to contrast-induced nephropathy. Chronic kidney disease is an important risk factor, and hydration is the mainstay of prevention. While inpatients can undergo intravenous hydration, limited knowledge exists regarding regimens for outpatients. We employed a rapid outpatient hydration protocol to reduce postponement of imaging appointments for patients with suboptimal estimated glomerular filtration rate (eGFR).@*METHODS@#From June 2015, we amended our CT preparation protocol to mandate rapid hydration (oral, intravenous or both) for patients with an eGFR of 30-60 mL/min/1.73 m@*RESULTS@#226 outpatients received the hydration protocol, which correlated with a 95% reduction in postponement of imaging appointments. No complications of fluid overload from hydration were encountered. A significant association was observed between age and decrease in eGFR, but this was not significant when stratified by drop in eGFR category. No statistical significance was found between decrease in eGFR and gender or race. Higher baseline eGFR was less likely to be associated with decrease in eGFR after imaging. Type of hydration was not related to a drop in eGFR category for patients with an eGFR of 45-59 mL/min/1.73 m@*CONCLUSION@#We defined a shorter hydration regimen that is safe to use in the outpatient setting.
